Would probably mean your upload directory/temp directory is not set correctly in phpCheck the php.ini and see if it is set to the right spot and the webserver has permission.

I have previous and next buttons enabled, but if the next or previous image is a "mature" image, it still shows the thumbnail. I believe the thumbnail ought to be blocked.
